    • Account Numbers

  • There are numerous ways for an identity thief to obtain your information. Most victims believe they have been jeopardized online. Although a thief can obtain your information online, information can be stolen much easier through low-tech means such as telephone scams or even un-shredded personal documents you throw away.

  • In the event that your identity has been jeopardized, follow the steps below:

    • Fill out the affidavit (PDF) provided by the Federal Trade Commission (FTC).
    • Compile documentation relating to your compromised identity (i.e. bank statements, letter from the Internal Revenue Service, credit bureau reports).
    • Bring in the affidavit and documentation to the Melissa Police Department or call at 972-838-2033. A report number will be generated and provided to you.
    • The case will be investigated or forwarded onto the proper authority.
    • Upon exhausting all leads, the case will either result in the perpetrator being found and charged or the case is closed.
